Form Requirements for Vendors
The State of Wisconsin requires all Stone Barn Craft Sale vendors to comply with state tax regulations:
- All vendors must complete Form S-240, Part C (Vendor Information Form) and mail it to the Town of Chase, 8481 County Road C, Pulaski Wi 54162.
- You must also complete the Rental Contract (Click on the button below)
